Home Visitation Programs

Des Moines County Public Health offers two home visitation programs to support families.

Family Support Home Visitation Program

The purpose of the Family Support Home Visit Program is to provide home visits by a Registered Nurse to pregnant women, new parents or families with children up to the five years of age.

Healthy Opportunities for Parents to Experience Success (HOPES)

We believe...

  • All families have strengths. Building upon these strengths allows families to make positive changes in their lives.
  • Preventing problems now is better than trying to fix them later. This is especially true when it comes to our children.
  • Parenting is the toughest, yet most important job anyone will ever have. All parents need support and encouragement as they care for their children.

Our Family Support Workers are qualified individuals that are trained to help with questions and problems parents may have. They will assist parents in caring for their children by making scheduled in-home visits. They understand the questions and stress that all parents have.

Prenatal families or families with newborns who reside in Des Moines County may be eligible for HOPES services. A Family Support Worker may work with a family until a child reaches five years of age or until the family no longer requests services.
